Botox or Fillers: Which Injectable Is Right for You?
If you've ever wondered whether Botox or dermal fillers would better address your skin concerns, you're not alone. It's one of the most common questions we hear at Dermatology of North Asheville, and understandably so. Both are minimally invasive injectable treatments that can help you look more refreshed and youthful, but they work in fundamentally different ways and are designed to treat different concerns. Here's what you need to know.
Two Different Problems, Two Different Solutions
The key to understanding Botox versus fillers is recognizing that not all wrinkles are created equal.
Dynamic wrinkles are the lines that form from repeated facial movements such as squinting, smiling, frowning, and raising your eyebrows. Over time, those repeated muscle contractions leave their mark on the skin. Botox is a neuromodulator, meaning it is a nerve-blocking treatment that keeps targeted muscles from contracting, which softens the appearance of these expression lines. Common treatment areas include the forehead, the lines between the eyebrows (often called "elevens"), and crow's feet around the eyes.
Static wrinkles are different. These are lines that are always visible, even when your face is at rest — the kind that appear as gravity takes over and your skin naturally loses volume and elasticity with age. This is where dermal fillers come in.
Dermal fillers are gel-like substances injected under the skin to smooth wrinkles, restore fullness, or improve contour. Unlike Botox, fillers don't affect muscles or nerves — they fill in areas that have lost elasticity or volume over time. Common treatment areas include the lips, cheeks, and the lines around the mouth.
How Each Treatment Works
Botox (Botulinum Toxin)
As we discussed in last month's blog, Botox is an FDA-approved injectable that temporarily relaxes targeted facial muscles. Results typically last 3 to 4 months and can be maintained with follow-up injections. Other FDA-approved neuromodulators work similarly and include Dysport®, XEOMIN®, and Jeuveau®.
Dermal Fillers
Dermal fillers such as Juvéderm and Restylane are made from hyaluronic acid, a natural component of the skin that has a plumping, softening, and hydrating effect. Depending on the type of filler used, results can last up to 24 months. Unlike Botox, filler results are often visible almost immediately.
Can You Use Both?
Yes, and many patients do. For static wrinkles, neuromodulators like Botox treat the underlying muscle movement while filler addresses the lines at rest. When used together strategically, the two treatments complement each other well and can produce more comprehensive results than either treatment alone.
Which One Is Right for You?
It depends on your specific concerns, anatomy, and goals. A few general guidelines:
If your primary concern is expression lines in the upper face (forehead, between the brows, or around the eyes), Botox is likely your best starting point.
If you're noticing volume loss, hollowness in the cheeks, thinning lips, or deeper lines around the mouth, a filler may be more appropriate.
If both are present, a combination approach may give you the most balanced result.

Both Botox and fillers are considered very safe when administered by a trained and licensed professional in a medical setting.
A Note on Safety
We want to emphasize something we touched on last month: where you receive treatment matters. Vascular occlusion (when filler is accidentally injected into a blood vessel) is a rare but serious risk that is far more likely when the injector is not properly qualified or trained. Always seek treatment from a board-certified dermatologist or other licensed medical provider who understands facial anatomy and uses FDA-approved products.
